Patient Information

Your first visit with Dr. Reddy as a new patient will be at one of our South Bend Orthopaedics offices. If you are unable to make your scheduled appointment, please cancel at least 24-hours in advance. Your courtesy will allow another patient to make an appointment during that time.

Please bring the following items with you to your appointment:

• Current Photo ID
• Current Insurance Card
• Acknowledgment Form
• All Previous X-ray, MRI, CT Scans and Report within One Year
• Written Referral, if required by your insurance policy
• Written Worker’s Compensation Authorization – if applicable

In addition, please fill out and bring the forms below with you to your first visit:

• Patient Health History
• Authorization for PHI Use and Disclosure Form

Also, please fill out and bring the disability index form below that corresponds to your condition:

• Cervical Myelopathy Questionnaire
• Lower Back Disability Index Form
• Neck Disability Index Form
